It is estimated that 70-80% of medical errors take occur when a patient is handed off or transferred to another caregiver or facility. These errors typically occur due to fatigue, recall issues, misreading illegible handwriting, misreading acronyms or being in a hurry to end his or her shift and hence missing to pass on critical pieces of information to the next caregiver.
